Application Process
New Families
If you are interested in adopting through New Life Family Services and this is your first time, please submit your Preliminary Application (pdf) along with the Parental Preferences Checklist (pdf) and a $50 non-refundable processing fee. Carefully complete all requested information as failure to do so will delay the processing of your application. Please note that both husband and wife must respond individually to the questions on the application. Preliminary Applications must be approved prior to being invited to one of our scheduled Information Meetings. Attendance at this meeting is required for all couples pursuing adoption through New Life.
Returning Families
If you have adopted through New Life in the past, and are interested in adopting again through New Life, please contact the Social Work Program Assistant at 612-866-7643 Ext 158. You will then receive the Application for Adoption of Additional Child and Parental Preference Checklist. Carefully complete all requested information as failure to do so will delay the processing of your application. Please note that both husband and wife must respond individually to the questions on the application. Your Application for Adoption of Additional Child must be approved prior to attending an Informational Meeting. Attendance at the Information Meeting is required with each new adoption.
